    Cancellation

    APPOINTMENT CANCELLATION POLICY AND CANCELLATION FEES

    This page is ONLY for appointment cancellations.  If you need to reschedule, please click HERE to Contact Us.

    • You MUST cancel at least 24 hours in advance of your appointment to avoid a fee.
    • Monday appointments (or Tuesday appointments when Monday is a holiday) cancellations must be made before 3 pm on the previous Friday to avoid a cancellation fee.
    • There is the following cancellation fee with less than 24 hours’ notice.
      • Current Patients: $105.00
      • New Patients (your first appointment with Dr. Grumet): $125
    • If you are a no-show for your appointment, there is the following missed appointment fee:
      • Current Patients: $105.00
      • New Patients (your first appointment with Dr. Grumet): $125
    • Early cancellations give us time to schedule an appointment with someone on the waiting list.

    If you cannot complete this form, please call the office to cancel at 404.685.9414.

    You will receive an email acknowledging your cancellation. You must keep this email as your receipt you canceled 24 hours before your appointment.  If you do not receive an email within 30 minutes of completing this form, we did not receive your cancellation (check your spam folder).  You need to call the office to cancel.
